For those trying to find the entry form, once you get past the age check, scroll down through all the brands until you see the box with "Multibrand" (Captaing Morgan, Smirnoff, Crown Royal) and click that box. That will take you to the offers page where you will click on the "Do Game Day Your Way Sweepstakes" box.
